  1. A legal birth certificate showing the child will be 4 on or before Sep.1, 2024,
  2. Clear copy of Medicaid card OR 2 proofs of income OR documentation of family experiencing homelessness, transiency, or in foster care,
  3. Two valid proofs of residency, ~~ We can accept utility bills, property tax bills, rental/mortgage agreements. Dates must be current. 
  4. AND a current South Carolina immunization record.
  • Be 4 years old on or before Sept. 1, 2024
  • Be eligible for the free- or reduced-price lunch program or Medicaid
